A 24 year old man suffers head, face and chest injuries from a gas explosion.

Primary Survey

Airway: Partially obstructed. Blood and debris in mouth an oropharynx. Blood from nose. Significant facial injuries.

Breathing:
Bilateral, widespread crackles. External grazes, lacerations and bruising
O2sats 89% on 100% oxygen.

Circulation:
Pulse 110/min, BP 180/90.

Dysfunction:
GCS 9(E1V2M6) Right pupil reactive. Left not visible.

Following a rapid sequence induction of anaesthesia attempts at orotracheal intubation failed due to inadequate vision from oral and pharyngeal haemorrhage.
