Despite multiple rib fractures this patient currently has no incdication for either tracheal intubation or tube thoracostomy, although she is at risk of respiratory compromise and needs frequent re-assessment.

The X-ray shows an abnormal superior mediastinum which warrants further investigation. Aortic arch angiogram revealed a contained aortic rupture at the level of the aortic isthmus. While angiography remains the gold standard for aortic injury, helical contrast enhanced CT scanning is increasingly used.
