The X-ray shows an abnormal superior mediastinum which warrants further investigation. Aortic arch angiogram revealed a contained aortic rupture at the level of the aortic isthmus. While angiography remains the gold standard for aortic injury, helical contrast enhanced CT scanning is increasingly used.
Despite multiple rib fractures this patient currently has no incdication for either tracheal intubation or tube thoracostomy, although she is at risk of respiratory compromise and needs frequent re-assessment. |
