43 year old builder. Lost control of van and drove it into a shop front. Initially awake and talking at scene but then loss of consciousness. Intubated by the paramedics. Lost blood pressure in the ambulance.

Primary Survey
Airway: Intubated and ventilated
Breathing: Trachea central. Poor air entry.
Circulation: Heart rate 20 on ECG monitor. BP unrecordable.
Dysfunction: GCS 3. Pupils equal. No external signs of head injury
Exposure: No other obvious external injury
