You get all the kit together for your tracheostomy, and make your incision. The small amount of blood around initially turns into a torrent as you incise the thyroid gland and you lose all your landmarks. Finally, with a lot of haemostats, ties and generous amounts of diathermy the blood flow abates. You identify the trachea, and incise it vertically. You pass the tracheostomy tube and listen for breath sounds.
