The Unit

Description

The Chatlos Trauma Center is the only Level I Trauma Center in Central Florida serving over 15 counties. It is also contains one of only four burn units in the state. Orlando Regional Medical Center (ORMC), located in downtown Orlando, is a major teaching hospital with 8 residency teaching programs and two fellowship programs. This 520 bed hospital is one of only 6 teaching hospitals in the state. Given the popularity and rapid growth of the region, the Trauma Program admitted over 3500 patients in 2000, a 300% increase from five years ago. The 30 bed adult ICU (10 surgical / trauma, 8 neuroscience, 8 medical, 4 burn) admitted over 2000 patients in 2000. A major ICU expansion project is currently underway which will double the number of ICU beds within the next 2-3 years.

Research Activities

A comprehensive clinical research program, supported by a full-time clinical research coordinator, provides the fellow with extensive research opportunities. Multiple clinical and industry-sponsored trials are in progress. ORMC was the first ICU in the United States to test the new continuous right ventricular end-diastolic volume pulmonary artery catheters. Current research interests include oxygen transport and pulmonary support, hemodynamic monitoring technology, new treatments for sepsis, high-level ventilatory support, and intraabdominal hypertension / abdominal compartment syndrome.

Fellowship Program

Clinical Responsibilities

The philosophy of the critical care training program is that a hands-on, direct patient care approach provides the optimal learning experience. The unit is staffed by surgical critical care fellows and general surgery and emergency medicine residents. Medical and physician assistant students are also assigned to the service. Call is taken in-house on an every third night basis by all members of the Critical Care Service. A graded level of responsibility is assigned, as the fellows become accustomed to the goal directed protocols. The fellow gains excellent experience in clinical management, problem solving, and bedside procedures (including endoscopy).

The second year trauma fellow is a junior faculty member of one of the trauma services at ORMC. The trauma fellow rotates with other faculty members in call responsibility with approximately one night per week of in-house call. In addition to honing skills in the surgical management of trauma, the fellow participates in administrative and quality improvement aspects of the trauma center, to round out their preparation to function as a leader in trauma systems upon completion of the fellowship.

Research Opportunities

Each fellow is encouraged to become involved in both existing as well as original research projects of their own design. Numerous clinical studies in progress are available to the fellows and presentation at regional and national meetings is expected. It is anticipated that each fellow will finish with at least one peer-reviewed publication per year of fellowship.

Teaching Responsibilities

The fellows are expected to participate actively in the teaching of both residents and students. Regular participation in conferences and delivery of grand rounds presentations is anticipated.
