Is the pt stable? If not then needle. If the pt is stable I’d say tube.

needle thoracostomy will buy time for few minutes, but i would say with advanced life support traing available in all e.r across the world, chest tube is preferrable as it hardly takes a few minutes under a trained hand.

How was it possible to obtain this image in a patient like this? is not a classical teaching that a tension pneumothorax is a clinical diagnosis?. I guess the cornerstone of a certified discussion should be this point and not the management. Management is clear in the hospital setting: if you can use the pleural tube, use it ASAP, do not give place before to the neddle.
