This X-ray shouldn’t have been taken, as we do not need X-ray to diagnose the problem.
The ankle should have been reduced prior to the X-ray.
Need operative fixation to achieve a good outcome in ideal circumstances.

I do not agree with the previous comment
As a x-ray will take only few minutes to take but would reveal valuable information such as associated injuries ( as in this case the fibula fracture)the outcome with the data obtained through x-ray would greatly improve the operative care that can be given to such patients.
The delay in obtaining the x-ray probably by few minutes would not be a waste or irreparable loss.
As for the definitive treatment, ankle has to be reduced under GA and fibula needs to be fixed preferably with a plate/screws.

In treatment of ankle Fracture-dislocation we should take in cosideration the soft tissue status as this would delay fracture fixation due soft tissue swelling development. Delaying reduction in this case until obtaining Xray would put the skin under risk and increase the swelling. Therfore I would agree with mukhp99 comment.

As soon as the patient presented in the E.R. with this kind of fracture dislocation, and fresh at that, reducing it even without any pain killers won’t bother the patient that much. Yes, an x-ray may be obtained but rectify the deformity initially and immobilize it if possible then do the x-ray. Afterall, surgery will still be required.
