Grade 4 pancreatic injury.
Adult male struck by a car at a high rate of speed. Closed head injury, left sided rib fractures with hemo-pneumothorax and bilateral tibia/fibula fractures. CT scan obtained. Operative findings were a transection of the pancreas right over the portal vein. Distal pancreatiectomy/splenectomy. Pt. had on peri-pancreatic fluid collection requiring percutaneous drainage and recovered.
