This patient sustained a gunshot wound to the head causing substantial tissue loss over the middle third of their face. On examination a partially thrombosed pseudoaneurysm of the brachiocephalic trunk was identified. Proximal vascular control was obtained and distal reconstruction carried out via a double pericardial patch. The patient underwent an uneventful recovery and left hospital at day 20 without complications.
