Full cervicosternotomy exposure revealing hematoma in the ascending and aortic arch ,innominate trunk and left common carotid artery. Innominate vein repair done with a running prolene 5-0 suture and the aortic arch with a pledget suture.

Excellent work Did you decrease bp the when you put the stitches in the aorta sometime it works better for the stitches tearing through in my experience and pericardial pledgets has helped me. Did you use any patch material on the vein

I used permisive hipotension for a aorta repair ,
also used pericardial pledgets ,an the vein in these case:primary repair , in other case repair with safena path .

Thank you, in this case we proceeded with immediate surgical exploration since the entry was presented with signs of arterial injury drives: active arterial bleeding for cutaneous wound and shock did not respond to correction of volume.Proceed with a cervico esternotomy for a full access .
