Shotgun wound to chest - lung injury at thoracotomy

Self-inflicted shotgun wound to the left hemithorax. Projectile exited posteriorly.
He was admitted in shock with a left chest wall haematoma, and fracture of four ribs.
At thoracotomy he was found to have an inferior left lobe laceration with cavitation and lung haematoma.
