I've never done a MASH type thorocostomy (using a Foley and two inverted IV 
 bottles), but I'll agree....a decent blade, a pair of Peon clamps, and 
some #1  silk on a straight needle, along with the drain system du jour, will 
handle many  if not most chest tube insertions.

We place a sterile disposable scalpel on top of the tray to prevent  the
delay & frustration of trying to attach the blade in  a  rush.
