I'm a bit late entering this thread but I wonder if one can be so cavalier in ligating the radial artery. The plastic surgery literature has shown early fatigue and effects on grip strength after the radial forearm flap. Ergo, this is not a disposable vessel. Therefore in a clean cut I would explore in the OR and would repair the artery if cut. Also I don't buy the "it'll thrombose anyway" defence. We do repair much smaller brachial arteries in kids' nasty supracondylars and they do just fine (well.. mostly). Of course a badly chewed up artery or a segmental loss would be another story. My 2 Rupees. And of course best wishes for a very happy, healthy and prosperous New Year to all my colleagues. Regards Sohail Muzammil
