Just for the record, and for those not familiar with DUSTOFF, ask any member of the armed forces and you will be told that DUSTOFF = angel = some of the best medical pre-hospital personnel you will ever meet!

in rural America, it is more based on the desire NOT to tie up  extremely 
limited, often critically needed, local assets for long  transports.
 
Some of these outfits cover 15,000-20,000 population counties with *two*  
staffed BLS crews, and have mutual aid (if available) response times 
approaching  one hour.
 
With some transfers, we often have 'lost' a ground unit for 3 to 5 hours,  
and on weekends, it has not been unusual to have two pending transfers 
awaiting  a: the return of a crew from a previous transfer, b: the arrival of 
mutual aid,  or c: weather clearing enough for a helo transfer.
 
And, as a former DUSTOFF medic, I have often been more  conservative of 
requesting helo transfers than other docs at a particular  facility.
