All,

?I know we have discussed this in the past but I am trying to get a general
consensus about DVT prophylaxis?in head injured patients.? Meeting with our
neurosurgeons later in the week and would like some input as the literature
is pretty sparse.? When is it ok to start LMWH in patients with SAH?? SDH?
stable exam and CT's?? thoughts?? What are you folks doing out there?

48 hours after injury / haenorrhage control. That is a totally random  
number, but one I could get all our neurosurgeons to agree to.

This is what we use here at BMC.  I personally use an IVF filters on all of
my spinal cord patients, and in spite of the lack of evidence to support my
belief, I think that the severe TBI's that most likely will not recover
functionally but that we keep going should also have an IVF filter, either
retrievable or permanent.  For patients with contraindications (absolute or
relative) to DVT prophylaxis I am quick to filter.

Just my 2 cents...
