Charging for FAST Exam

Hello all,

A quick question (mostly for the US users as it relates to billing under Medicare rules) -- does anyone on the list bill for their FAST exams and if so do you have a routine report/dictation/or form that you use? We just bought a new US that lets us print the exam and it seems silly not to report and bill it if we can.
