Prioritizing injuries

I would like to thank you in advance for you time. I would like to  
hear from some of you how you prioritze injuries in the setting of the  
unstable multi-system trauma patient.  In particular, a patient   
presents with aortic dissection, open book pelvic fracture and CHI-- 
which procedure is of priority?  Do you take them to the OR to graft  
the dissection or to angio to embolize the pelvis?  If angio first to  
stop the bleeding, when do you take them to the OR to correct the  
dissection?  This patient also had a significant CHI including  
subdural and Laforte 3 fracture. Is your first priority ICP  
monitoring?  What SBP goal would you try to achieve to maximize CPP  
while at the same time preventing more bleeding by "popping the  
clot"?  This is in the setting of blunt trauma.
