Maybe the mid 1980s? ABCDE was solidly in place when I took  my first ATLS 
class at Ft Sam Houston, TX in the Summer of 1988, and they were  talking 
about (other) upcoming changes in the 'new edition' which was due out in  
1989, IIRC.

As I reacall the D and E, was an   outgrowth of the ATLS committee of the 
ACS during the late  1980s.
