Doc,

Many trauma patients are intoxicated with alcohol or drugs at the time  
of their injury. It is felt that this intoxication often is a  
contributing factor in their injury. It is also felt that if the  
frequency or degree of intoxication could be lessened, the chance of  
re-injury could also be lessened. Using a very short screening tool  
like CAGE is helpful because it requires only asking 4 simple  
questions and does not require any "intervention" skills on the part  
of the provider who asks the questions. (C = ever try to Cut back?, A  
= have you gotten Angry if friends or family have talked to you about  
your drinking/drug use?, G = have you felt guilty about your drinking/ 
drug use?, E = do you ever use an "eye opener" to get going in the  
day?).

Similar short screens exist for domestic violence and there are other  
screens for ETOH/drugs. Screening only makes sense if you can then  
offer an a more detailed interview and/or intervention by someone  
trained to do it safely.
