OK, gotcha now. Yes this is a primary care concept, in that we are all primary care physicians when we accept the care of any patient, and need to treat any and all diseases to the best of our (or our consultants') abilities. And so, if I admit a trauma patient who was driving while intoxicated, or fell down a flight of stairs with A BAC of 476 (a gent that I just actually counselled a few minutes ago), I need to sew up the lacerations, have the ortho guys fix the broken bones and have the neurosurgeons comment on the small SDH. And then, before I discharge the patient someone (and yes it often is me, as that patient's physician) needs ot try and prevent that patient from becoming a patient again. The ACS COT has mandated that we, as trauma surgeons and our hospitals, as verified trauma centers, screen ALL trauma admission for substance abuse; while initially it was alcohol, due to the frequency of alcohol-related injury, but it will soon be all substance abuse. Once the screening has turned up a positvie result, we need to then determine if there is a problem (i.e. chronic abuse) and if so treat the problem as we would any other disease. Hope that helps.
