I am aware of trocars and forceps iatrogenically injuring the abdominal aorta by general general, surgeons, gynecologist, neurosurgeons, orthopodist, and others. I am aware of the thoracic aorta being stabbed by trocars and large catheters by general surgeons, emergency physicians, thoracic surgeons, anesthesiologist, and others.

I have heard of more organs removed thinking it was the gallbladder.

One could speculate that this was a trochar punching a hole in the aorta without the surgeon being able to fix it and ligation of the aorta was done to stop the blood loss.
