I use Erythromycin frequently in the ICU, I don't like metoclopramide anymore. I have had a pretty good success with the Emycin.

do you ever use Erythromycin as a prokinetic?

I've had a number of patients with somatic dyskinesia from
metoclopramide...and yes, most of them also react to the phenothiazines in a similar way.

Yes, if I need to. First check that there is no mechanical obstruction,
correct all electrolyte abnormalities, stop all constipating drugs, then
try all other types of prokinetics. If all else fails - Neostigmine. Use
in standard dose mg/kg alone, in ICU on full monitoring - risk of
dysrhythmia. If no response - try one more time. If that does not work you
may need to try other options.
