abdominal trauma lecture

Hi there,

I am a military paramedic with the British Royal Marines and a Critical Care Practioner and specialist in advanced trauma.

We have a quick way to deal with abdo trauma in the battle field particularly with visceration. This is only used by specialist paramedic's and Special Forces medics. As usual we will follow the all the normal protocols i.e CABC or MARCH depending on time constraints. The original Idea was dervived from Dr J Hagman.

If we have time and the casualty is stable we will obviously check the exposed intestine. If the testine is bleeding and damaged we will simply dress it with mild compression and a wet Adbo dressing.

However if undamaged we will carefully put the intestine back into the cavity and on completion open a bag of warm Saline 0.9% and poor some of it into the cavity whilst patient is on his back. Once complete we will then place 3 rapid stitches into the wound as a basic closure. Once this is done and all are happy, WITH ASSISTNCE we will gently log roll the patient on to his or her side thus allowing the saline to drain out. This in turn keeps the intestine wet and some element of sterility.

This method has been used on several occasions and has saved life and intestinal tissue. THis is something that is not recognised in normal civillian paramedicine but on the battle field with the terrible casualties we are seeing then it is a valuable drill and I will use it again.
