Vedr.: TEG forTrauma Patients

Since 2004, here in Copenhagen, we have with great succes used TEG on all
bleeding trauma activations (approx 20% of all 1000 activations/year)
sampled at the time of arrival blood work-up. We run it in the blood bank,
and can either see it on a windows interface in the trauma bay or get TEG
guidance from Blood Bank Services after 15 min of assay time. Transfusions
is done by 1:1:1 and guided solely by TEG repeated every hour or in
agreement with blood bank.

Approx 20-30% of bleeders have significant coagulopathy requiring
intervention by FFP/PLT/antifibrinolytics etc. allready on arrival

Who has experience with the TEG for trauma patients? Our cardiac surgeons
have been using it for three years and we are trialing it for managing
major trauma resuscitations. Preliminary results are very promising. We
also have been impressed with its ability to detect hypercoaguable states
and to focus VTE prevention measures.
