Textbook example of missed injury.
I would attempt closed reduction, reorganise the frame to perform a DHS fixation given the young patient. Would have a low threshold to do it open. Best regards, Zsolt Balogh

Dear colleagues!

Male, 44 y.o. Trauma in MVA 22.08.08: open fracture of right femur
(middle shaft), close fractures of right elbow and pilon.
He was in rural hospital were debridgement and Ilizarov frame for femur
were performed. Now, he is in Regional Trauma Hospital. After 18 days of
trauma x-ray of pelvis were performed - fracture of right femur neck.
Wounds at femur is healing with small points of necrosis.
I'll be very appreciate for your suggestions and opinions about further
treatment!
