Giles, you might search the archives of the critical care mailing list, as  
we had that discussion a year or so ago. The situation there was a long term  
expedition of Naval Cadets sailing (literally) around the world. How big a crew 
 and how long are you looking to stock for? Also, what kind of training will 
your  sick berth attendant have, and what kind of communications will be  
available?
 
Other sources would include Auerbach's Wilderness Medicine, Chapman's  
Piloting, The Diver Alert Network, and the Wilderness Medical Society.
