Trauma Center Level Categorizations

Good Evening,

I am currently writing a EMT textbook and would like to place a brief table
explaining the levels of trauma centers and the criteria that entitles them
to that level.

I have searched the ACS site for some kind of table, something simple for
the EMT level that they will understand the levels and how it relates to
their
destination decisions easily.

Does anyone know where I could find a table or list of criteria I can make
into a simple, quick look table for the book?
