Recently had a fit adult admitted following penetrating injury to the right chest with injury of the lower lobe of the lung extending to the hilum with associated extra-pericardial pulmonary vein laceration. Had a right pneumonectomy as very coagulopathic with oversew of the pulmonary vein. Initially did well, extubated at 24 hours, comfortable, haemoserous drainage from chest tubes. 12 hours later after a couple of transient dips in saturation developed acute pulmonary oedema, froth coming up the ET tube, and died within minutes.
