TRAUMA AND SURGICAL INTENSIVIST COVERAGE

CC is a seperate sub-speciality in South Africa, with either 2 years pure
critical care after a 5 year surg or any other residency, combined across
the board, with a single multidisiplinary exit exam (Cert Crit Care SA) or
as a one year trauma / one year crit care Trauma Surgery seperate option
as from
November 2007 (i.e. still getting off the ground - first fellowships will
start only in 2009 and anyone so-far registered is by grand-father clause
incl. yours truly) and everyone (even non surgeons) must be competent in
trauma and will do at least 3-6 months in a Surgical ICU. The combined CC
cert. allows up to 15 months in the ICU of the parent discipline (e.g.
surgery), but everyone (incl the trauma surgery option) will do kids and
adult medical rotas so as to manage the comorbidities. We are less
inclined to "consult" due to shortages of medical personnel in South
Africa and must be able to do a bit of everything (e.g. we do our own
dialysis).

This may be the model to strive for, but I suspect the legal issues will
once again intervene on that side of the Atlantic.
