Anthony,
The problem isn't so much the bleeding, but rather the enzymatic
auto-digestion of the peri-pancreatic tissues after a pancreas is
injured but not noted on CT or PE - especially  after blunt injury.

Ron, is it because the bleeds tend to be slow and are well hidden by
the pancreas itself? If there is bleeding is it venous ?

Unfortunately, it is NOT an uncommon occurance - there can indeed be
pancreatic injury not noted on CT and not noted on initial PE, but one
that manifests later on in the course of the day, 12 - 24 hours, in my
experience. I guess now the question I would ask is just what are you
