Trauma Critical Care course materials

I again was not able to attend the Trauma-Critical Care course.  As in past 
years I wait for the course materials to become available for sale and I 
purchase the course syllabus from the Trauma-Critical Care web site.

Unfortunately this year I am having great trouble. I carefully enter my 
information and the transaction gets denied but no specific reason is given. 
I am only given a long list of "possible" reasons and I am asked to 
carefully re-enter the required information.

This has been done VERY CAREFULLY for several attempts, all without success.

I have tried to repeatedly e-mail the web site with my concerns but so far 
no responses.

Sorry to hit the list with this but if Dr. Mattox is out there....HELP!!!
