My opinion is 3 Qualified elsewhere that the patient is intubated and may be the reason for the score of 1 for verbal. I deal a lot with Trauma Registry Data points. There is only space to have an actual number in calculating the GCS. There are other places to include whether the patient was sedated, intubated, or what ever else may be a reason for the final scoring.
