If the patient clearly needs the OR, then I would say however long it takes you to call the OR to tell them you are on the way up plus transit time to the OR. As I see it, that would be under 10 minutes in my place. Now, if you want the standard ACS COT party line, the ED stay for a severely injured patient should be under 30 minutes.
