What I'm getting at: was this a prolonged crushing event, or did he just get the stuffing knocked out of him? Without plethora or petechiae (at minimum), I'm thinking the first diagnosis to exclude would be that of traumatic asphyxia. Frankly, I've read almost nothing in the thread thus far that distinguishes this as traumatic asphyxia, at least not over other garden variety blunt chest trauma. Consult William of Occam.
