It's also sad because the no-fault quest for perfection in the airline industry is continually held up to us as worthy of emulation. I wonder how long that would have lasted if every employee who caused a near-miss tragedy apologized to everyone on the plane! I imagine they would have needed statutory protection. Sentinel events, root cause analyses -- how are we to do a meaningful quest for quality when we're threatened with litigation every time someone catches a problem?
