Ian,
I am certainly no expert, but as I see it, the bilateral incision (ie extension of the lt thoracotomy across the sternum to the right) gives you much better visualization of the entire heart, and enables you to perform open compressions more easily while doing whatever the repair de jour you need to do.

We've been through this before, but could an expert surgeon please
explain specifically the pros and cons of left anterolateral thoracotomy
versus bilateral clamshell thoracotomy.
