It's tempting to politely ignore this perennial declaration of yours, Dr. Mattox; but there are earnest and impressionable readers out there who need to be reminded that, however occasionally, our heroes can be obstinate -- and wrong. This is not a matter of opinion. IO access has discrete logical, ethical, clinical, and traumatic indications. Rapid vascular access in hostile environs is one, as I believe our military colleagues would agree. And in pediatric resuscitation, where access is harder than many of us would admit, a few seconds of fruitful IO insertion beats several minutes of hopeless poking around. Abuse is relative. Ask Al Gonzales. I frankly agree with you in this given scenario, having doubts that most civilian blunt trauma benefits in any important way from access, much less fluids; but not all emergencies are surgical, and even surgical emergencies can give rise to parallel medical crises (inadequate pain or airway control as examples). Your passion is perilously narrow-minded on this topic, and risks contaminating the dialogue. Doesn't make you less of a hero in my view, but please remember that you must use your powers only for GOOD. Unless one is attempting a cruel form of child abuse, or adult abuse. Especially today when it is acknowledged that both for children, teen agers, adults, etc., permissive hypotension and restrictive (to no) fluid resuscitation is better than the old way, IO completely looses its market and appeal.
