Rob, Thanks for taking the time to reply, especially with the research that you put into it. I am familiar with the protocols, and if you are in Iowa, I probably know your facility. As you know, all of Iowa hospitals are designated as trauma care facilities. There are only two level ones: the University in Iowa City and Iowa Methodist in Des Moines. Level two centers must have neurosurgeons as well as other specialists. Level three facilities must have general surgeons. Level fours must staff an ED, which can be with mid-level providers. For various reasons, including an unwilling med staff, some places have specialists, but are categorized at lower levels. Iowa City is 100 ground miles from Keokuk. With the proper indications, air transport is the best way to go. The questions remain: what were the injuries, what were the indications for transfer, who has authority for transfer, and what were the benefits and risks of the managment selected. EMS is represented on the local trauma committee, which will meet next week.
