Bill, the bad news is that so far the hospitals are always the one who blinks first! So who is the dumb one here? The one with the good life, the luxury car and the $1,000 suites, or the one with the moral high ground and the ethics that keeps him/her in the ED where he/she is needed! Don't get me wrong - I have no complaints about the way I live. However, I do have complaints about sharing the title "doctor" with those who take a lot of money from the hosptials to take call from home, sleep at home while they are on call, and allow their PA's to do all of the procedures that the on call doctor should have done after - and hang on to your hats for this one - SEEING the patient with their own eyes and EXAMINING the patient with their own hands. That is, after all, what doctors were taught to do, right? RIGHT???? I have to tell y'all - there really are times I am embarrassed to call myself "doctor" when I have to share the title with "those guys"! What DOES happen depends on how much power the hospital has over the doctor in question vs. how much money the doctor brings into the hospital. For example, if there are 2 or more hospitals in town a busy surgical group who has privileges at both has a great deal of negotiating power with the hospital for if the hospital revokes their privileges, they will take their cases across town. That's why our neurosurgeons and orthopods get paid more for a night of call through which they sleep than I do for resuscitating their head injury and polyorthotrauma patients. It often turns into a game of chicken between the hospitals and the docs.
