Dear Andrew J Bowman
TO die or allow to die have a distinct different connotation in the English 
language, that you know better than me. "To allow to die" is a world tragedy 
since very old times. No one aspects that in the US people die because they 
have no insurances, but sadly it happens ... as in any poor nation.
May be the costs of excessive malpractice insurance and payments is choking 
the system, may be is the uninsured foreigners, may be is a lack of a real 
medical system...... you will know for better than me what will be the 
answer.
I, as many people in the world hopes that the US can keep their  prevalent 
scientific and medical position and hope for a fast recovery of the illness 
affecting the heath system.

  No one is denied health care there, no one is dumped to another hospital 
or allowed to die in an ER.
