Sounds like a severe blunt cardiac injury. Do you have a transesoph. echo? Did the BCI somehow cause a intraventricular (cardiac) thrombus that embolized? Or, did a piece of plaque from his aorta go north? Would get the TEE if you don't have one...it will tell you a lot, I think.
