About a year ago, I saw a woman in the ED who developed bilateral Phlegmasia rubra dolens, had a Greenfield filter, and was not anticoagulated. I started her on heparing and consulted vascular surgery, and the good doctor Rama took her to the OR for thrombectomy. I don't know how she fared. The lady had Protein C deficiency, which I see not infrequently in Afro-americans in Detroit. There is a lot of litigation in this neck of the woods due to missed pulmonary emboli, and I think docs are nervous. There certainly isn't much science out there on this, is there? DB
