Jago, Go to Pub Med and type in aminophylline and asystole. Interesting data, keeping in mind that ROSC in no way implies successful cerebral resuscitation. Anecdotally (the best kind of evidence:))I have given it 3 times in 3 years, and have had temporary ROSC once (eventual re-arrest and termination). I do NOT routinely give this drug in an arrest. Interestingly, the European Resuscitation Council has it in their 2005 guidelines (ref follows). I cannot tell the indication from the abstract, but would assume they are referring to asystole. Anaesthesist. 2006 Sep;55(9):958-66, 968-72, 974-9. Aminophylline would not have a parasympathetic effect on the cardiovascular system. On the contrary, it is a methylxanthine and would be expected to produce a sympathomimetic effect. Hope this helps.
