DVT surveillance

What is the standard of care for DVT surveillance AFTER placement of an
IVC filter?  If periodic Doppler studies are continued, and a clot is
found after filter placement, what treatment is undertaken in a patient
who had a filter placed because they could not be anticoagulated?
