OUCH!  Looks like that would hurt.

As a non-orthopod, but having seen this before, I would think that an IM rodding of the humerus and plating of the scapula would be the right thing to do.  Once that is done the clavicle almost becomes a non-issue and could be treated as most others - non-operatively.  Once post-op, however, I would guess that the patient is going to have to be completely non-weightbearing for a bit - and I would ask our ortho colleagues what that time frame is.
