Peter Was the hernia repair open or laparoscopic? Did he look "bled out" - one of the possibilities is a femoral arterial injury with retroperitoneal blood tracking causing this pain picture. Let us know what the autopsy shows! Since you are also from South Africa, feel free to mail me off-list if there is "local" related issues you may need advice on in this case.
