Role of trauma surgeon in a third world country can be extrapolated from
our own urban trauma centers. Example: Guys working in South of Bronx,
East of LA or the poorest big city in the entire USA (Cleveland).
 
   ROLE OF AN URBAN TRAUMA SURGEON     "You get there in less than
fifteen, fix/damage control every thing that can kill your patient and
wait for the Ortho guy to drive in early next afternoon, in his Porsche,
for his block time". Most trauma surgeons in these poor areas (Third
world in the first world) have no back up,CT, vascular, GU or finantial.
All injuries in initial 8 hours are fixed by the trauma
surgeon(Exception neuro).

  ROLE OF THIRD WORLD TRAUMA SURGEON     Difference in the first world
and the third is poor people. Most urban Trauma surgeons work in areas
where there are nothing but poor and therefore, are third world trauma
surgeons. Now if they are in a developing country and maintain a
franchised ACS certification and acceptable outcome, with added training
in basic ortho and trauma neuro can provide excellent trauma care to
there poor people. 

They might have better out comes, and cost less (by getting rid of the
Porsche and the block time) in the First World to!.

Providing care has got to be survival first, pretty picture later. The
funds should be appropriated for survival with the least amount of loss
from the patient. There are plenty of Nip/Tucks out there to correct the
image. If you do not look at,for example,thoracoabdominal with
penetrating trauma with upward trajectory and diaphragmatic tears with
pleural involvement without the worry of a damn scar who will perform
restoration? No one. That is why there is massage therapy! A Tucker
