A dilemna that is getting increasingly significant in Detroit (elsewhere?) is the availability of Plastics/Hand surgeons willing to take hand call for the Emergency Department. The reasons for this are unimportant. But, even orthopedists are shying away, and general surgeons are either already overworked, uninterested, or under-trained (I really don't know which - maybe all 3). Although, apparently, there is talk of a subspecialty of surgery-trained doctors who would be hospital-based and perform such duties (intermediate trauma surgery, if you will), I suspect this would be financially problematic in our current system in the States, and that there would be few takers for the job. I am curious if there would be support from surgeons for a hand fellowship for EM-trained physicians. It seems to me that motivated EM physicians are poised to fill this void. All comments welcome.
