Dear list:
   
  I have been out of the country for a few months, and also merely reading the posts for some time.   When I comment, it seems that I always offend someone, and that is not my intention.    
   
  I was trained to do general, trauma, critical care, thoracic, pediatric, and vascular surgery.   One of the greatest lessons I had to learn here in Los Angeles was that for elective surgery I must call a surgical specialist.   In the middle of the night those surgical specialists are not as willing to come to the hospital.
