Who should perform vascular surgery for trauma?

Your either trained well and know what your doing, or your not.  Many  have 
probably seen the paper of how to make an emergency Burr Hole with a  
screw-driver,, you might not want to take that on in your community hospital,  however, 
you may need to someday in Antarctica.  If you know what your  doing and the 
time is not an issue, pursue the vascular repair. If you dont know  what your 
doing and you can ship the patient out, ship them.  If you have a  choice of 
having to perform an amputation or trying your best based "on your  past 
training" and the time will not threaten the patients stability, I don't  think 
there is a question you should pursue a repair.  This question is  somewhat 
similar to community doctors on who should perform a whipple.   Doing what you feel 
would best serve the patient answers this  question, however, there will 
always be critics based on outcome on both  sides of this question.  DN
